Due to continued concerns about the spread of COVID-19 Eye Surgical has implemented CDC screening protocols. Patients will be required to wear a mask while in the office. If a mask is not available one will be provided to the patient. Only essential guests are allowed in the office. If the patient is accompanied by other family members they will not be allowed in the office unless they are essential to the visit (parent to child, caregiver, etc.) All essential guests are required to wear a mask at all times. Shields or any other device are not permitted.

Please be aware that you are submitting a request only. Until you receive either an e-mail from one of our schedulers or a telephone call, you do not have an actual appointment. To protect your health information you should not state anything that reveals your diagnosis or actual eye issue.

Keep your request simple, i.e. "I am requesting an appointment for my annual eye exam due this month or the best days for me to see the doctor is a Monday or Friday".
